Using a xbox 360 controller I have x,y,b assigned to keyboard 1,2,3.
I want to use RB to modify them to be 4,5,6 while RB is held.
I assigned RB to use set 2 when held and assigned x,y,b to 4,5,6 in set 2 it looks like it works in the xpadder itself but when I go to use it in the game it still uses 1,2,3
Any suggestions?

Open notepad and test the input there. If it doesn't work correctly there too, please post your profile, by opening the profile menu (the one on the right side) , clicking on "Copy" and pasting the profile here in a new post.

There is no problems with the profile. The only problem I could think of is that you were in Set2 when starting the game, so pressing RB would lead you from Set2 to Set1 instead of the other way around. Minimizing Xpadder before starting the game would automatically make sure that Xpadder is in Set1 (presumed that "Return to Set1 after minimize/restore" is still activated in the Xpadder settings), or of course you can also switch back manually before opening the game.
Alternatively you can have a look at the two possible while held glitch workarounds here, that would automatically to the correct direction when pressing the button.
